Signature Authorization Form
Current signature authorization forms on file are valid through December 31, 2007.
As of December 3, 2007 the Signature Authorization Form has been replaced by the Administrative Task Authorization Tool. This tool will allow department heads to delegate authority online. Department Heads, according the University MUD Table, should have recieved an email from the new tool detailing the necessary steps to delegate authority. The Signature Authorization Form is used to verify that signatures on financial and administrative documents are properly authorized. Each department, identified by 5 digit dept code, must complete a new form each year whether or not there have been changes.
Below you will find a link to the Signature Authorization Form. You may type directly in the form and print it. You may also print the blank form and fill it out by hand. After it has been signed by the appropriate personnel please return the original form to Nancy Patrylak, Unit 6076.
Once received, the form will be verified against the University MUD Table for a matching Department Head. If you'd like to check to see who is identified as the Department Head in the MUD Table go to the Budget Website. If a change is required, please complete the MUD Table Change Request Form on the website.
(It is required that departments use the form below. Older forms or alternate forms will not be accepted. )
